The Department of Education has released the DepEd Order Number 69, series of 2016, which is Provision of the Deped Electronic Record and Form 137 for public schools’ Senior High School as part of school permanent records.
The two mentioned template sheets are tools to manage and organize the learners’ records to help the teachers to compute the students’ grades per quarter and semester and records the students’ historical academic and co-curricular performances.
Below, I have provided the two templates of the report card in PDF that can be downloaded straight from my DepEd Google Drive. These are for free from our Department, and they are NOT FOR SALE. Please make use of them, fellow teachers.

Thanks for having this. Since the DepEd website migrated, we lost the link to the F137 download. However it’s good that it’s not in pdf as you have mention but in xls format 😉
